#  腎臟病病友 少喝火鍋湯

每到春節假期，腎臟病病友便需要面臨美食誘惑，往往較辛苦。腎臟內科主治醫師蔡慧芳建議，腎臟病病友對於鉀、鈉攝取量，都要特別注意，以火鍋湯來說，火鍋湯有鉀離子、鈉離子、普林等，且火鍋湯多半很油，建議腎臟病病友盡量少喝火鍋湯。

蔡慧芳也提醒，像蜜餞、果汁等，鉀離子含量很高，腎臟病後期病友，也盡量不要吃蜜餞、果汁。

**鉀離子過多 影響病友心跳**

蔡慧芳醫師解釋，腎臟病病友因為腎功能受損，鉀離子較無法排出體外，鉀離子持續累積，神經功能就會受影響，此時心跳容易變慢、過慢，嚴重時，心跳甚至可能停掉，造成危險，因此腎臟病病友也要多留意鉀攝取量高低。

美國腎臟基金會（American Kidney Fund）表示，腎臟病病友在疾病早期，可能不太需要特別限制飲食，就算有限制，也較寬鬆，當腎臟病逐漸惡化，可能就要開始限制鉀、磷等攝取量。

**哪些是低鉀食物？**

很多食物都含有鉀，身體需要足夠的鉀，肌肉才能順利運作，但對於腎臟病病友來說，鉀攝取量太多、太少，都可能造成問題。美國腎臟基金會指出，像蘋果、葡萄、鳳梨、草莓等，或者像花椰菜、洋蔥、蘿蔔、胡椒、萵苣等，都屬於低鉀蔬果，可以運用這些蔬果取代香蕉、甜瓜、橘子、柳橙、李子、葡萄乾、菠菜、蕃茄等高鉀蔬果。

對於腎臟病患者來說，白麵包、白米飯、牛肉、雞肉等，鉀含量也都比較低，而豆類食物、糙米等，鉀含量較高，攝取要有所節制。

**磷攝取量 也要多注意**

除了鉀，磷也是重點。美國腎臟基金會指出，磷會與鈣質、維生素D協同作用，幫助維持骨骼健康，腎臟健康時，可以調節體內磷濃度高低，當腎臟功能不佳，磷就會堆積於血液當中，一旦血液中的磷含量過高，骨骼就會變得很脆弱、容易斷裂。像義大利麵包、法國麵包，或者像玉米、原味、未加鹽爆米花等，都屬於磷含量較低食物，可以適量攝取。

此外，醫師有時也會建議腎臟病病友攝取維生素D、葉酸、鐵劑等營養補充品、補充劑，幫助預防一些腎臟病常見併發症。每個人狀況不同，醫師、營養師多會針對個別需求，給予適當建議。(蔡慧芳, 衛生福利部雙和醫院腎臟內科主治醫師)
