35歲王先生工作繁忙，應酬喝酒是家常便飯。半年多來，經常疲累、口渴，體重一下掉了20公斤，卻以為只是工作太累，不當回事。沒想到有天突然昏倒送急診，檢查發現血糖高達1200mg/dl，超過正常值10倍，原來早已罹患糖尿病卻不自知，因沒有就醫治療而引發急性高血糖，還好及時搶救，撿回一條命。

**初期無明顯症狀 不能靠三多判斷**

糖尿病就像隱形殺手，初期症狀不明顯，很容易輕忽，黃建寧指出，尤其正在打拚事業的青壯年族群，往往疏於照顧健康，就算血糖出現異常，也認為沒症狀就沒事，沒有進一步就醫治療，因此，大約有一成患者，確診時已經產生小血管病變等併發症。

**控制好飯後血糖也非常重要**

不少患者以為，只要空腹血糖正常就算是控制得宜，其實空腹血糖只能反映採血當下的血糖狀態，控制好飯後血糖也非常重要。黃建寧醫師認為，飯後血糖偏高會增加血管內的氧化壓力，造成內皮細胞發炎，導致心肌梗塞、中風等心血管疾病，也會提高視網膜病變、腎臟病變等慢性併發症風險。因此患者自我監測血糖時，最好餐前、餐後各量一次，如此可確實掌握食物對血糖的影響，適時調整飲食內容，血糖控制成效會更好。

**根據患者狀況搭配使用藥物**

降血糖藥物種類繁多，各有其療效和副作用，黃建寧醫師表示，醫生會根據患者狀況搭配使用，例如愛吃澱粉類、飯後血糖偏高的患者，就適合能抑制食物中澱粉及雙醣類水解的alpha-葡萄糖苷酶抑制劑，以延緩腸胃道中碳水化合物的消化與葡萄糖的吸收，使飯後血糖值趨於平穩，不會忽高忽低，降低對血管的衝擊，減少心血管併發症的風險，糖尿病前期的人使用，還能預防進展成糖尿病。

**血糖控制是一輩子的功課**

糖尿病是全身性的代謝異常疾病，無法治癒，如果只靠藥物，若完全不改變生活型態，治療效果有限，黃建寧提醒，就算有服用降血糖藥物，也不能隨便大吃大喝，特別是工作繁忙，社交活動又多的年輕患者，很容易不知節制而加重病情。黃建寧醫師強調，年輕病人的未來還很長，更要積極控制血糖，避免演變成失明、洗腎。
